This is a really great idea to have on your car windscreen. 
As a personal choice I have an active “Do Not Resuscitate” form. My body undergoes so much illness/stress every day, my heart is not healthy, my lungs do suffer and I struggle to keep my brain oxygenated as it is. As a family, we decided with the help of my medical team, that trying to resuscitate me is no longer a viable option for my quality of life. It could potentially save my life, but my quality of life would be severely impacted.

Having something like this on my car windscreen would not only make emergency services aware of my medical state, but would also alert them to focus on stabilising other passengers before me – as my chances of survival would be less that my fellow passengers. For me and my family, this is another way of making services aware of my condition, but at the same time protects my fellow passengers.
